Importance of PET/CT with [¹⁸F]-Fluorocholine in the Diagnosis of Primary Hyperparathyroidism in a Patient with Negative Parathyroid Scintigraphy with [⁹⁹ᵐTc]Tc-MIBI – Clinical Case

Abstract

Primary hyperparathyroidism (pHTP) is an endocrine pathology in which imaging techniques takes a crucial role in better therapeutic guidance. Parathyroid Scintigraphy with [99mTc]Tc-MIBI and Positron Emission Tomography/Computed Tomography (PET/CT) with [18F]-Fluorocholine ([18F]FCH) are useful Nuclear Medicine tools for studying parathyroid pathology. The reported clinical case concerns a patient with suspected pHTP whose [99mTc]Tc-MIBI scan concluded the absence of uptake in the parathyroid gland. A PET/CT with [18F]FCH was suggested, which successfully identified hyperfunctioning parathyroid tissue. PET/CT with [18F]FCH has been shown to be a promising technique in the detection and localization of hyperfunctioning parathyroid tissue, allowing to overcome false-negative cases resulting from [99mTc]Tc-MIBI parathyroid scintigraphy.
